By submitting this form, I acknowledge that I am providing certain Personal Information (“P.I.”) to Ordo Templi Orientis U.S.A. (“O.T.O.-U.S.A.,” commonly referred to as “U.S. Grand Lodge” or “U.S.G.L.”). I authorize O.T.O.-U.S.A. to maintain this P.I. in a restricted-access online database for the purpose of registering to attend NOTOCON, the National O.T.O. Convention of U.S.G.L. I understand this P.I. includes all of the information included in this registration form, including but not limited to:

•    My name, including my full civil name and any nickname or magical name I use to identify myself within O.T.O.-U.S.A. e.g. on my NOTOCON badge

•    My email address

•    My Local Body Affiliation

•    My phone number

•    My O.T.O. degree


I understand that O.T.O.-U.S.A. will use appropriate security technology and best practices to keep all P.I. confidential and only permit access to individuals who need it for use in connection with NOTOCON registration.


I acknowledge that technology is constantly evolving and there is no security technology that is 100% perfectly secure. I further acknowledge that neither O.T.O.-U.S.A. nor any other individual or entity can guarantee the non-disclosure of electronically stored information.  Specifically, I understand there will always be some risk of disclosure of any electronically stored P.I. and my association with O.T.O.-U.S.A.


I hereby indemnify and hold harmless Ordo Templi Orientis (which I may know as “O.T.O. International” or “I.H.Q.”) and O.T.O.-U.S.A. from any damages or claims which might arise from inadvertent disclosure of my electronically stored P.I.
